Practice rules

Welcome to 3D Tandarts. To provide safe and appropriate treatment, we apply the following practice rules. By making an appointment, you agree to these rules.

Registration and identification

  • At your first visit, we ask for valid identification (passport, identity card or driving licence) and your health insurance details. We are legally required to verify your identity and BSN.
  • Always inform us of changes to your address, telephone number, email address, insurance or health, including medication, allergies and pregnancy.
  • At your first visit, you complete a medical questionnaire. Accurate information is important for safe treatment.

Appointments

  • Please arrive on time. If you are more than 10 minutes late, the treatment may not be able to proceed in full.
  • If you cannot attend, cancel by telephone or email at least 24 hours in advance.
  • Appointments that are missed or cancelled too late may be charged based on the time reserved. These costs are not reimbursed by your insurer.
  • If you repeatedly fail to attend, we reserve the right to end your registration with the practice.

Fees

  • Our fees are based on the maximum rates set annually by the Dutch Healthcare Authority (NZa). Laboratory and material costs are listed separately.
  • For treatments costing more than €250, or when laboratory costs apply, you receive a written estimate in advance. We start treatment only after your approval.
  • Whether and how much your insurer reimburses depends on your policy. We recommend checking this in advance. You remain responsible for payment.

Payment

You can pay in the following ways:

  • Directly at the practice by debit or credit card.
  • Through Infomedics: our billing agency submits the claim directly to your insurer or sends the invoice to you by email or post.
  • Pay invoices within the period stated. For late payment, Infomedics may charge reminder and collection costs.
  • For invoice questions, contact Infomedics using the details on the invoice or contact our practice.

Treatment and consent

  • We explain the treatment, alternatives, risks and costs in advance. You decide whether to proceed.
  • Children under 16 attend with a parent or guardian.
  • After treatment you receive aftercare advice where necessary. Follow this advice and contact us if you have concerns.

Urgent care

  • For pain or an accident, we try to help you the same day. Call as early as possible: 010 890 6536.
  • Outside opening hours, please contact Dental 365 on 0900-1515.

Privacy and patient record

  • We handle your data carefully in accordance with the GDPR. Your record is retained for 20 years as required by the WGBO.
  • You have the right to access and receive a copy of your record. Data is shared only with your consent or on a legal basis, for example with your insurer or a referring practitioner.

At the practice

  • Set your telephone to silent in the treatment room. Filming or photographing staff or other patients is not permitted.
  • We expect respectful behaviour. Aggression, threats or discrimination are not tolerated and may lead to removal from the practice and a police report.
  • Preferably enter the treatment room without a companion, unless accompaniment is necessary or agreed.

Complaints

If you are not satisfied with your treatment or our service, please tell us. Our dentists will discuss your complaint with you at the practice or by email at klachten@3dtandarts.com. Together we will look for an appropriate solution.

If we cannot resolve the matter together, you may contact the KNMT (Royal Dutch Dental Association) through knmt.nl.
